Job Summary
Coordinate patient transitions from hospital and long-term care to primary care services across five outpatient Family Health Centers. Manage post-acute care for high-risk patients at risk of poor outcomes, frequent ER visits, and readmissions by completing post-discharge telephonic follow-ups within 48 hours. Facilitate medication reconciliation, care plan development, and education while assessing educational needs for patients and caregivers. Monitor quality of interventions using data from medical records and program metrics to set specific goals. Provide clinical mentorship to nursing staff through orientation, coaching, and participation in quality improvement activities aligned with National Standards of Care.
Required Qualifications
- RN
- three to five years relevant clinical nursing experience
