Job Summary
Triage Hospice and Home Health Registered Nurse RN; After-hours triage responsibilities including taking calls from hospice and supportive care patients, evaluating and counseling, and determining if a home visit is needed. Documents care provided and time spent on phone for triage, tracks problems and trends for staff education. Requires RN license in Missouri and Illinois with ability to obtain the other state license within 3 to 6 months; 2 years of Hospice experience; Home Health experience is a plus. Schedule includes 40 hours with some weekends and nights; weekends may be remote or in office with 12-hour shifts on weekends and 3 holidays per year. Provides patient care counseling under physician direction, supports hospice and home health services, and participates in broader home care programs. Required Qualifications
- RN license in Missouri and Illinois
- Must have at least one state and obtain the other state RN license within 3 to 6 months
- 2 years of Hospice experience
- Home Health experience is a plus
- Nursing Diploma/Associate's
- Bachelor's Degree in Nursing preferred
- Cardiopulmonary Resuscitation (CPR)
