Job Summary
Coordinate referrals for medical, dental, vision, and behavioral health services while monitoring their completion and communicating with parents regarding appointments. Facilitate workshops and classroom presentations on topics including emotional regulation, bullying prevention, and mental health awareness to support student wellness. Build positive relationships with students and families to provide social-emotional support and assist those experiencing behavioral or school-related challenges. Maintain accurate service logs, track referrals, and ensure timely documentation in compliance with HIPAA and FERPA regulations. Serve as a liaison between schools, healthcare providers, and families to promote a safe, supportive, and healthy school environment.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Counseling, Human Services, Family Science, Public Health, Education, Sociology, or equivalent work experience
- Minimum of one (1) year of experience working with children, adolescents, schools, healthcare, behavioral health, or community programs
- Strong interpersonal and communication skills
- Ability to build relationships with students and families from diverse backgrounds
- Proficiency in Microsoft Office and electronic documentation systems
Desired Qualifications
- Experience in a school-based setting or Federally Qualified Health Center (FQHC)
- Bilingual (English/Spanish/Arabic) strongly preferred
- Experience facilitating youth groups or classroom presentations
- Knowledge of trauma-informed care, HIPAA, FERPA, and mandated reporting requirements
- CPR and First Aid Certification
