Treatment Nurse - Licensed Vocational Nurse (LVN) (86640)

Provide quality of care to prevent and promote healing of alterations in skin integrity based on resident assessments and individual plans of care. Dress wounds and ensure timely dressing changes as prescribed, while monitoring healing progress and recommending adjustments when needed. Perform and record weekly pressure ulcer assessments, document audit results on the Weekly Focused Skin Assessment, and communicate findings to the DON, attending physician, and responsible parties. Participate in training related to skin care and rotate on-call schedules as assigned by the DON or Administrator. Assist with other nursing functions as requested. Active RN/LVN license in Texas required; wound care certification preferred. Previous skilled nursing experience is preferred.
