Job Summary
Treatment Director, Complex Medical Needs oversees nursing assessments, service planning for medically fragile children in foster care, and leads liaison efforts with home health agencies and hospital discharge coordination. The role provides after-hours phone support, collaborates with physicians, case managers, and foster parents to ensure comprehensive care, conducts initial health screenings at the residential facility, and ensures quarterly, aligned care plans with the child’s permanency goals.
Required Qualifications
- Bachelor’s degree in nursing from an accredited school
- Licensed Registered Nurse in good standing with the Texas Board of Nursing
- Certified CPR and First Aid Instructor or willing to obtain certification within 90 days of employment
- Experience conducting patient assessments, screenings, and ongoing health evaluations
- Experience supporting individuals with mental health and behavioral needs using trauma-informed care practices
- Experience coordinating services with social workers, physicians, and community resources
