Job Summary
Coordinate and monitor individualized, active treatment plans for individuals in ICF/IRA settings; lead the Interdisciplinary Team to implement and adjust plans, advocate for individuals, and ensure programming aligns with regulatory requirements. Develop and update service plans based on individual preferences and needs, conduct assessments, set milestones and life goals, and create staff action plans to ensure continuous active treatment. Collaborate with advocates and providers, manage referrals and service delivery across Day Habilitation and residential settings, document and maintain records, monitor satisfaction and safety, and address concerns with providers or advocates. Provide in-service training, model independence, and supervise staff to support optimal potential and normal growth for individuals served. Maintain compliance with funding sources and participate in regular plan reviews, ensuring timely distribution of plans and addenda to relevant parties.
Required Qualifications
- Master’s Degree in a Human Services field and one (1) year of providing services to persons with Developmental Disabilities
- Bachelor’s Degree in a Human Services field and three (3) years of experience in providing services to persons with Developmental Disabilities
- valid driver’s license
