Job Summary
Assess emotional, financial, spiritual, educational, and social needs of residents and families; develop discharge plans and arrange post-discharge follow-up care. Document progress at interdisciplinary care conferences, conduct family counseling meetings, and act as advocates to ensure residents exercise their rights. Interview clients regarding lifestyle adjustments, coordinate transportation and appointments, and serve as a liaison between departments, families, and outside agencies. Assist in pre-admission processes, support new hire onboarding, and maintain effective communication with healthcare professionals and facility staff. Requires active Social Work license, prior long-term care experience, and ability to travel up to 100% to assigned centers across five states.
Required Qualifications
- Bachelor's degree in Social Work
- Active Social Work license in good standing
- Minimum of one (1) year of supervised social work experience in a healthcare setting, working directly with individuals
- Prior experience in long-term care
- Must be able to travel up to 100% to assigned centers, including travel by car or air
