Job Summary
Assess emotional, financial, spiritual, educational, and social needs of residents and families to enhance quality of life through individual and group interventions. Document progress, update care plans at interdisciplinary conferences, and develop discharge plans while arranging post-discharge follow-up services. Act as a resident and family advocate, conduct family counseling meetings, and serve as a liaison between departments, outside agencies, and healthcare professionals. Assist with pre-admission processes, onboarding new hires, and training staff. Requires a Bachelor's degree in Social Work, active license, and extensive travel to assigned centers across five states.
Required Qualifications
- Bachelor's degree in Social Work
- Active Social Work license in good standing
- Minimum of one (1) year of supervised social work experience in a healthcare setting, working directly with individuals
- Prior experience in long-term care
- Must be able to travel up to 100% to assigned centers, including travel by car or air
