Job Summary
Assess emotional, financial, spiritual, educational, and social needs of residents and families; conduct one-on-one counseling and group work to enhance quality of life. Document progress at interdisciplinary care conferences, arrange transportation and appointments, and act as a liaison between departments, families, and outside agencies. Develop discharge plans, organize family counseling meetings, and maintain effective communication with healthcare professionals and facility staff. Assist in pre-admission and admission processes, including training new hires. Requires extensive travel up to 100% to assigned centers across multiple states.
Required Qualifications
- Bachelor's degree in Social Work
- active Social Work license in good standing
- Minimum of one (1) year of supervised social work experience in a healthcare setting, working directly with individuals
- Prior experience in long-term care
- Must be able to travel up to 100% to assigned centers, including travel by car or air
