Job Summary
Assess and support the emotional, financial, spiritual, educational, and social needs of residents and families through individual, group, and interdisciplinary interventions. Conduct one-on-one counseling, arrange transportation, and manage appointments while acting as a liaison between departments, families, and outside agencies. Document progress, update care plans at interdisciplinary conferences, and lead Family Counsel Meetings. Develop discharge plans and coordinate post-discharge follow-up services. Interview residents to gather behavioral and lifestyle history, assist with onboarding new hires, and advocate for resident rights. Requires a Bachelor's degree in Social Work, active license, and extensive travel up to 100% across assigned centers.
Required Qualifications
- Bachelor's degree in Social Work
- active Social Work license in good standing
- Minimum of one (1) year of supervised social work experience in a healthcare setting, working directly with individuals
- Prior experience in long-term care
- Must be able to travel up to 100% to assigned centers, including travel by car or air
