Job Summary
Assess and support the emotional, financial, spiritual, educational, and social needs of residents and families through individual, group, and interdisciplinary interventions. Interview clients to understand past and present adjustments, document progress at care conferences, and develop discharge plans while arranging post-discharge follow-up. Act as an advocate for residents and families, organize family counseling meetings, and serve as a liaison between departments, outside agencies, and facility staff. Travel up to 100% to assigned centers across skilled nursing and long-term care facilities to ensure continuity of services. Maintain effective communication with healthcare professionals and assist in training new hires.
Required Qualifications
- Bachelor's degree in Social Work
- Active Social Work license in good standing
- Minimum of one (1) year of supervised social work experience in a healthcare setting, working directly with individuals
- Prior experience in long-term care
- Must be able to travel up to 100% to assigned centers, including travel by car or air
