Job Summary
Assess and support the emotional, financial, spiritual, educational, and social needs of residents and families through individual, group, and interdisciplinary interventions. Conduct pre-admission assessments, interview clients to understand lifestyle adjustments, and develop discharge plans while arranging post-discharge follow-up care. Organize Family Counsel Meetings, maintain care conference documentation, and act as a liaison between departments, outside agencies, and families. Travel up to 100% to assigned centers across skilled nursing and long-term care facilities to ensure service continuity. Requires a Bachelor's degree in Social Work with an active license and at least one year of healthcare experience.
Required Qualifications
- Bachelor's degree in Social Work
- Active Social Work license in good standing
- Minimum of one (1) year of supervised social work experience in a healthcare setting, working directly with individuals
- Prior experience in long-term care
- Must be able to travel up to 100% to assigned centers, including travel by car or air
