Job Summary
Travel Skilled Nursing Facility (SNF) Physical Therapist needed for a 13-week assignment in Alamosa, CO. Provide 1:1 PT services to Medicare patients in a SNF, evaluate and treat, develop and update individualized plans of care (POC), collaborate with nursing, OT, SLP, and rehab leadership, document evaluations, progress notes, and discharge summaries, and educate patients and families on mobility, safety, and recovery. New Grad PTs are welcome with mentorship available.
Required Qualifications
- Doctorate or Master’s Degree in Physical Therapy
- Active (or pending) state PT license
- SNF experience preferred, not required
