Job Summary
Travel SNF Physical Therapist responsible for evaluating and developing treatment plans for geriatric patients, executing restorative and maintenance programs, documenting treatments, collaborating within interdisciplinary teams, and floating to nearby facilities as needed. Position is a 13-week travel assignment in Lakeside, AZ, with a 4x8 schedule (40 hours/week), 32 hours guaranteed, and benefits discussions with the recruiter. Requires a PT degree (BA/BS/DO) with a current Arizona PT license and BLS, prior SNF/long-term care experience, and familiarity with equipment and interdisciplinary collaboration.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Arizona State Physical Therapy license required to start the assignment
- BLS (Basic Life Support) certification from American Heart Association or American Red Cross may be required
- Experience working in a skilled nursing facility or long-term care setting is required
- Experience providing care to geriatric patient populations, including restorative and maintenance programs
- Demonstrated ability to document skilled treatments, develop individualized care plans, and work within interdisciplinary teams
