Assess patient physiologic and psychosocial health status through interviews, chart reviews, and clinical observations of gastrointestinal, cardiovascular, respiratory, renal, and neurological systems. Formulate and implement individualized plans of care, prioritizing activities, administering medications safely, and performing grade I-IV decubitus care. Collaborate with physicians and the care team to coordinate treatments, delegate assignments, and provide discharge planning and education. Document all assessments, interventions, and outcomes accurately while maintaining constant vigilance for patient safety and responding rapidly to emergencies. Requires an Associate's or Bachelor's degree in Nursing, 1-2 years of experience, and a current RN license in New York State or Connecticut.
