Job Summary
Conduct in-home patient evaluations and functional assessments for post-surgical recovery, fall injuries, and chronic conditions within the Franklin/Macon County area. Develop individualized treatment plans, deliver therapeutic exercises, and manage a caseload of approximately 5–6 patients daily while educating caregivers on home exercise programs. Maintain accurate clinical documentation in HomeCare HomeBase and coordinate care with interdisciplinary teams. Requires a minimum of one year of home health experience, an active North Carolina license, and reliable personal transportation for patient visits. Schedule is Monday through Friday, 8:00 AM to 5:00 PM, with 40 hours guaranteed per week.
Required Qualifications
- Minimum 1 year of Home Health Physical Therapist experience
- Active North Carolina Physical Therapist License
- Strong communication and documentation skills
- Reliable personal vehicle for patient visits
- Candidates must be comfortable working independently in a home health setting
- Strong time management and organizational skills required
- Reliable transportation is essential for patient visits
Desired Qualifications
- Experience with HomeCare HomeBase (HCHB)
- Previous travel experience
