Job Summary
Conduct in-home patient evaluations and functional assessments to restore mobility and improve functional independence for post-surgical, chronic, neurological, and orthopedic conditions. Deliver therapeutic exercises, mobility training, and rehabilitation interventions while developing individualized treatment plans and educating patients on home exercise programs. Complete OASIS documentation accurately and coordinate care with physicians and interdisciplinary teams to ensure optimal patient outcomes. Manage daily visit schedules efficiently, maintaining compliance with all documentation and regulatory standards. Requires a minimum of one year in home health, an active California PT license, and BLS certification.
Required Qualifications
- Minimum 1 year of Home Health experience
- Active California Physical Therapist License (or proof of application at submission)
- OASIS experience
- BLS Certification (AHA)
- Valid driver's license and reliable personal transportation
- Strong communication and documentation skills
- Strong time management and independence required for field work
- COVID-19 Vaccine (Medical/Religious Exemptions Allowed)
- Flu Vaccine (Medical/Religious Exemptions Allowed)
Desired Qualifications
- Previous travel experience
- Familiarity with home health workflows and scheduling
- Experience managing independent caseloads
- CA license preferred in hand at submission (pending license accepted if already applied)
