Job Summary
Travel Physical Therapist to provide home health services from Kingston, NY with an office base and coverage across Ulster County. 14-week assignment, 40 hours/week, 8-hour shifts, travel within Kingston and surrounding areas. Responsibilities include delivering home-based PT to patients recovering from injuries and managing chronic conditions, coordinating care with office staff, and supporting patient independence. Benefits include mileage reimbursement, potential housing assistance, and license reimbursement. Requires NY PT license, degree in physical therapy, and preferably Homebase scheduling/documentation experience; BLS may be required.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active New York State Physical Therapist license
- BLS certification from AHA or ARC may be required
- Home health experience required
- Experience using Homebase is preferred
