Travel Physical Therapist (Home Health) — Virginia | 13-Week Contract

Conduct physical therapy evaluations, reassessments, and discharge visits in patients' homes throughout the New River Valley. Develop individualized plans of care based on patient goals and educate families on therapeutic exercises, mobility training, and fall prevention. Collaborate with physicians and interdisciplinary teams while completing documentation electronically using WellSky on a company-issued tablet. Provide skilled services for a manageable caseload with one-on-one patient care, ensuring efficient travel between geographically scheduled visits across Blacksburg, Christiansburg, Radford, and surrounding communities.
