Job Summary
Conduct comprehensive patient evaluations in the home setting and develop individualized treatment plans to restore function following illness, injury, or surgery. Provide therapeutic exercises, educate patients and caregivers on safety practices, and monitor progress to adjust rehabilitation goals as needed. Collaborate with physicians and interdisciplinary teams while maintaining accurate EMR documentation. Complete 25 visits weekly for guaranteed 40-hour day shifts (5x8), with mileage reimbursement and possible on-call rotation. Requires active California PT license, BLS certification, and valid driver's license. Flu and COVID vaccines required; home health and OASIS experience preferred.
Required Qualifications
- Active California Physical Therapist License (or pending license with proof of application)
- BLS Certification (AHA)
- Valid driver's license and reliable personal transportation
- Flu Vaccine Required (Medical or Religious Exemptions Allowed)
- COVID Vaccine Required (Medical or Religious Exemptions Only)
- Requested Time Off (RTO) must be submitted during application
- Home Health checklist completion required before start
Desired Qualifications
- Home Health experience preferred
- OASIS documentation experience preferred
- Strong time management and independent clinical skills
