Travel Physical Therapist(Glendale, CA)

Conduct comprehensive patient evaluations in the home setting and develop individualized treatment plans to improve mobility and functional independence. Provide therapeutic exercises and mobility training while monitoring progress through regular reassessments. Educate patients and caregivers on home exercise programs and safety practices, then maintain accurate documentation in the EMR system. Coordinate with physicians, nurses, and interdisciplinary teams to ensure continuity of care across the Palmdale and Lancaster territories. Complete 25 visits per week over a 13-week contract, with guaranteed hours once the visit quota is met. Flu and COVID vaccines are required, and on-call rotation may be discussed during the interview.
