Job Summary
Travel Home Health Physical Therapist to perform admissions, recertifications, discharges, evaluations, and routine visits in patient homes within the coverage area; deliver home-based PT services, collaborate with interdisciplinary teams, and support patient independence and recovery. Requires driving to patient homes in Virginia Beach/Norfolk/Portsmouth areas, active VA PT license, and BLS; mileage and travel expenses reimbursed; 17-week assignment with 40 hours/week and a shift pattern aligned to typical clinic hours while providing comprehensive evaluations and plan of care.
Required Qualifications
- Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Virginia Physical Therapy license required prior to the start of the assignment
- BLS certification from the American Heart Association or American Red Cross may be required
- Home health experience required
- Ability to perform admissions, recertifications, discharges, evaluations, and routine patient visits in home settings
- Reliable transportation for patient visits and willingness to be reimbursed for mileage between visits
