Travel Physical Therapist for a 13-week contract in Sacramento, CA. Responsibilities include evaluating referred patients for rehabilitation, developing and administering treatment plans under physician orders, documenting patient progress, and educating patients and families. Requires graduated from an accredited program, active or pending PT licensure, and CPR certification. outpatient-focused rehab with 40 hours/week and 8-hour days.
Required Qualifications
- Must have graduated from an accredited school
- Current state license as a Registered Physical Therapist / PT or actively pursuing licensure
- Current CPR certification
- Develop a plan of care for each physical therapy patient
- Provide skilled physical therapy services / interventions in accordance with physician orders
- Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions
- Collaborates with all disciplines to plan and evaluate team goals for each patient
