Evaluate and treat adult and geriatric patients in a skilled nursing and senior living environment, developing individualized treatment plans focused on functional mobility, balance, strength, and safe discharge planning. Document care timely and accurately in the facility's EMR while collaborating with nursing, physicians, and other rehab disciplines to optimize patient outcomes. Educate patients and families on home exercise programs and safety. This 13-week contract includes 8-hour day shifts with 40 hours per week, no on-call requirements, and an expected 86% caseload. The setting offers strong exposure to geriatric and medically complex patients within a supportive, efficient workflow.
