Job Summary
Travel Physical Therapist (PT) for a 13-week contract in Scottsdale, AZ. Provide direct patient care in SNF/LTC settings, assess referred patients, establish and administer PT treatment programs, document patient conditions, and educate patients and families. Must be licensed as a Physical Therapist (or actively pursuing licensure) with CPR certification. Duties include developing plans of care, delivering skilled PT interventions under physician orders, and collaborating with the interdisciplinary team to meet rehabilitation goals.
Required Qualifications
- Graduate from an accredited physical therapy program
- Current state license as a Physical Therapist (PT) or actively pursuing licensure
- Current CPR certification
- Develop a plan of care for each physical therapy patient
- Provide skilled physical therapy services under physician orders
- Assess patient needs, plan, evaluate and modify care to meet PT goals
- Collaborate with interdisciplinary team to plan and evaluate goals
- Travel/Contract assignment
- Shall provide direct patient care and documentation as part of standard PT duties
