Job Summary
Assess patient needs, develop treatment plans, and document progress for patients in a skilled nursing facility while collaborating with healthcare professionals to help them regain mobility and independence. Requires a current Connecticut PT license and clinical experience in skilled nursing or rehabilitation settings. Must be able to hit the ground running with 13-week duration, 40 hours per week, and 8-hour day shifts.
