Job Summary
Travel Outpatient Physical Therapist needed for a 13-week assignment in Toledo, IA. Responsibilities include providing comprehensive outpatient PT evaluations (60 minutes), developing and implementing individualized treatment plans, documenting care in accordance with clinic and regulatory requirements, and contributing to performance improvement initiatives. Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program (APTA approved)
- Active Iowa state physical therapy license
- BLS certification may be required
- Experience providing outpatient physical therapy and 60-minute evaluations
- Clinical documentation skills including goal writing and documentation
- Ability to develop, implement, and modify treatment plans and supervise/lead therapy services
- Adherence to state practice act and continuing education requirements
