Provide evidence-based care to medically complex, high-acuity patients in a long-term acute care setting. Perform comprehensive assessments, medication and IV administration, and manage ventilator and wound care while monitoring patient status closely. Collaborate with a multidisciplinary team to develop and implement care and discharge plans, maintain accurate EHR documentation, and uphold strict infection-control and safety standards to support exceptional patient outcomes.
