Provide evidence-based care to medically complex, high-acuity patients in a long-term acute care setting. Conduct comprehensive assessments, administer medications and IVs, manage ventilator and wound care, and monitor patient status. Collaborate with a multidisciplinary team to develop and implement care and discharge plans, maintain accurate EHR documentation, and uphold strict infection-control and safety standards while supporting exceptional patient outcomes.
