Job Summary
Provide skilled nursing services and coordinate home health care for assigned patients in San Antonio, including medication administration, wound care, and therapeutic treatments. Perform nursing assessments, develop care plans with physicians, and re-evaluate patient needs using the OASIS data set. Manage care by supervising LVNs and home health aides, documenting communications, and planning patient discharge. Requires on-call rotation for weekday and weekend shifts with mileage reimbursement and shift differential. Must hold an unrestricted Texas RN license, one year of home health experience, wound care skills, and proof of required immunizations.
Required Qualifications
- Graduate of an accredited school of nursing
- Current unrestricted Texas RN license or compact license
- 1+ years of experience in home health care or related environment
- Wound Care
- Developed computer skills
- Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
- Tuberculosis screening
- Proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease
- Bilingual (Spanish)
- Required on calls (weekday and weekends)
