Job Summary
Deliver skilled nursing care under RN direction by performing focused patient assessments, administering medications safely, and executing nursing interventions with minimal supervision. Identify and report significant clinical changes, actively contribute to care plan revisions, and evaluate patient responses to treatment. Supervise unlicensed staff by delegating tasks, providing real-time guidance, and ensuring compliance with regulatory standards. Reinforce discharge planning, participate in interdisciplinary conferences, and communicate care recommendations to patients and families. Maintain medication security, adhere to infection control protocols, and report safety concerns per facility policy.
Required Qualifications
- Graduate of an accredited practical or vocational nursing program
- Active, unrestricted LPN license in the state of employment
- Demonstrated experience in skilled nursing, rehabilitation, or geriatric care
- Current BLS/CPR certification (per facility/state requirements)
- Ability to communicate clearly and professionally in English
- Compliance with TB screening and occupational health requirements
