Travel LCSW - Full Time Contract

Job Summary
Travel LCSW with full-time contract — provide licensed clinical social work services for hospice and outpatient bereavement programs across home settings, skilled nursing facilities, assisted living, and adult foster homes. Responsibilities include advocating for patients and families, coordinating resources, delivering grief and loss counseling, and collaborating with interdisciplinary hospice teams to support patient care. Work within hospice and community settings, performing duties such as pain and symptom management support, medical social services, counseling, spiritual support, bereavement coordination, and coordination of hospice aides. Requires a Bachelor’s degree in social services, counseling, or psychology; licensure as an LCSW and BLS; experience or education in grief counseling; and willingness to travel to various care settings with occasional weekends.
