Travel LCSW - Full Time Contract

Job Summary
LCSW needed for a full-time contract in travel-based hospice care. Provide home-based hospice and palliative support in the Lacey, WA area, advocating for patients to access resources and address psychosocial needs. Responsibilities include clinical assessment and care coordination for a caseload of ~50–60 patients, utilizing HCHB, and delivering patient- and family-centered social work in a home-health/hospice setting. Requires MSW, active LCSW license in WA, and travel within Lewis or Thurston Counties. Typical schedule is Mon–Fri, 8:00 AM–5:00 PM (40 hours/week) with benefits, housing options, travel and license reimbursement, and relocation assistance.
Required Qualifications
- Master’s degree in social work (MSW) from an accredited program
- Current LCSW or equivalent state clinical social work license in good standing with Washington State licensing board
- HCHB (Homecare HomeBase) experience required
- Experience in hospice, palliative care, or home health settings preferred
- Caseload management experience preferred (50–60 active cases)
- Two years of supervised clinical experience preferred
