Job Summary
Assess patient needs and develop individualized hospice care plans in collaboration with physicians and care teams. Administer pain management interventions, medications, and treatments to ensure patient comfort while monitoring changes in patient condition. Educate and support families and caregivers regarding symptom management and end-of-life care. Document all care provided, symptom changes, and family interactions accurately and in compliance with hospice regulations. Coordinate care with interdisciplinary teams including social workers and chaplains to provide holistic support. Ensure adherence to infection control, safety, and healthcare regulations while meeting visit and productivity goals.
Required Qualifications
- One year of recent experience as an RN
- minimum of six months in a hospice setting as an RN
- Valid RN license in compliance with state regulations
- BLS certification (AHA/ARC)
- Critical thinking
