Job Summary
Assess patient needs and develop individualized hospice care plans in collaboration with physicians and care teams. Administer pain management interventions, medications, and treatments to ensure patient comfort while monitoring condition changes. Educate and support families on symptom management and end-of-life care, documenting all interactions and interventions accurately. Coordinate with interdisciplinary teams including social workers and chaplains to provide holistic support. Adhere to infection control, safety, and regulatory requirements while completing charting and productivity goals. Perform other duties as assigned within the scope of practice.
Required Qualifications
- One year of recent experience as an RN
- minimum of six months in a hospice setting as an RN
- Valid RN license in compliance with state regulations
- BLS certification (AHA/ARC)
- Critical thinking
