Travel Home Health Speech Language Pathologist - $2,167 per week

Conduct one-on-one, evidence-based speech-language pathology services for patients in their home or community settings across Honolulu and Oahu. Perform comprehensive evaluations for adults and older adults recovering from stroke or neurologic conditions, developing individualized plans of care and setting functional goals. Deliver skilled interventions promoting communication and swallowing safety while educating patients and caregivers on home exercise programs. Travel between patient homes daily to manage a caseload that supports focused, unhurried care, collaborating closely with nursing, physical therapy, and occupational therapy teams. Document evaluations, daily notes, and discharge summaries using an electronic medical record system with support from clinical leadership.
