Job Summary
Travel Home Health Physical Therapist needed in Sonora, CA for a 13-week assignment. Provide home-health therapy, assess and evaluate patients’ functional status, develop plans of care, and complete OASIS assessments as appropriate. Manage a caseload of at least six patients per day in patients’ homes, utilize Kinser EHR, and collaborate with interdisciplinary teams. Requires a Physical Therapy degree (PT) and an active California PT license; BLS may be required. Travel benefits include housing assistance when available and license reimbursement; pay is based on a 40-hour week for 13 weeks.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active California Physical Therapy license required to start the assignment
- BLS certification from AHA or ARC may be required
- Experience in home health settings, including assessing and evaluating therapeutic/rehabilitative/functional status and developing the plan of care
- Experience completing comprehensive OASIS assessments when appropriate
- Experience evaluating home environments and documenting care using electronic systems; familiarity with Kinser preferred
- Ability to manage a caseload of at least six patients per day
- Current resume required to submit for consideration
