Travel Home Health Physical Therapist - $2,579 per week

Conduct comprehensive physical therapy evaluations, develop individualized treatment plans, and deliver therapeutic interventions including strengthening, balance, gait, and transfer training to adult and geriatric patients in their homes. Perform home safety assessments, recommend adaptive equipment, and educate patients and caregivers on exercise programs and fall prevention. Manage daily caseloads involving post-orthopedic recovery, stroke, cardiac, and chronic disease management while documenting visits in an electronic medical record system. Travel between patient homes in the Cedartown, Georgia area during daytime shifts, coordinating care with nursing, occupational therapy, and speech therapy teams.
