Travel Home Health Physical Therapist - $2,466 per week

Conduct one-on-one physical therapy services for adult and geriatric patients recovering from surgeries, orthopedic injuries, and neurologic events in homes throughout Eufaula, Alabama. Perform comprehensive assessments, develop individualized plans of care, and implement treatment interventions focused on improving mobility, safety, strength, and independence. Complete in-home evaluations, design therapeutic exercise programs, provide gait and balance training, and coach patients and caregivers on home safety and fall prevention. Collaborate with interdisciplinary teams including nursing and occupational therapy to ensure coordinated care while documenting visits in an electronic medical record system. Travel between patients within an assigned territory to support thorough, high-quality care with adequate time for education and hands-on treatment.
