Travel Home Health Physical Therapist - $2,432 per week

Conduct one-on-one skilled physical therapy services in patients' homes throughout Lewistown, Pennsylvania, performing 5–7 visits daily. Deliver comprehensive evaluations, develop individualized treatment plans, and implement interventions for diagnoses including post-surgical orthopedic conditions, balance disorders, and neurologic impairments. Focus heavily on functional mobility training, transfer techniques, gait and balance work, strengthening, endurance training, home exercise programs, and fall prevention strategies. Collaborate with an interdisciplinary team including nurses, occupational therapists, and social workers to ensure cohesive care, while documenting visits and care plan updates in an electronic medical record system.
