Job Summary
Travel Home Health Physical Therapist (PT) needed in Nutley, NJ. 13-week travel contract offering 40 hours per week with 5–6 patient visits daily (45–90 minutes each) and travel time factored into productivity. Responsibilities include performing initial and ongoing PT assessments, developing individualized plans of care, providing interventions for strengthening, endurance, pain management, and fall prevention; conducting gait and balance evaluations, home safety assessments, and mobility training; educating patients and caregivers on exercise programs and safety strategies; collaborating with RNs, OTs, SLPs, and physicians; and documenting all care per Medicare, OASIS-E, and agency standards. Qualifications include an active PT license, current BLS, 1+ year home health experience, experience with PT evaluations and therapy planning, familiarity with Medicare home health guidelines and OASIS-E, EMR experience (Homecare Homebase, NetSmart, Epic), and a clean background/driving record with reliable transportation. Required Qualifications
- Active, unrestricted Physical Therapist (PT) license in the state of assignment
- Current BLS certification (AHA)
- 1+ year of recent home health experience (within the last 2 years)
- Experience conducting PT evaluations and developing therapy plans
- Familiar with Medicare-certified home health guidelines and OASIS-E documentation
- Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
- Clear background check, driving record, and up-to-date health clearance (TB, immunizations)
- Reliable transportation with valid auto insurance and ability to travel between patient homes
