Job Summary
Conduct evaluation and rehabilitation intervention in the home setting, including management and supervision of patient care and family education. Establish plans of care to meet patients' needs in the home environment, coordinate with other disciplines, and provide direct patient care based on established care plans and physician orders. Apply evidence-based practice guidelines, Medicare regulations, and Conditions of Participation while modifying treatment frequency and duration as appropriate. Master CMS OASIS scoring and collaborate with the Quality team to adjust assessments. Acquire and maintain understanding of Home Health insurance requirements. This 13-week travel assignment in Missoula, Montana, includes 40 hours per week across 8-hour shifts.
Required Qualifications
- BS degree or better from an accredited Physical Therapy program
- current license to practice in the State of Montana
- valid driver's license
- appropriate auto insurance
- 2 years of previous PT experience
- PT(Compact Accepted)
- BLS(Copy needed)
- SSN Required
- DOB Required
- Work History: Should not be more than 30 days of gap in last 1 year
- References: 1 Reference in entire work history
