Job Summary

Travel Home Health Physical Therapist in Easton, MD for 13 weeks, providing patient-centered home health care. Responsibilities include performing initial and ongoing PT assessments, developing individualized plans of care, delivering skilled interventions for strengthening, endurance, pain management, and fall prevention; conducting gait and balance evaluations, home safety assessments, and mobility training; educating patients and caregivers on exercise programs and safety strategies; collaborating with RNs, OTs, SLPs, and physicians to ensure coordinated care; and documenting all care per Medicare and agency standards. 40 hours per week, 8-hour shifts, with travel to patient homes. Required Qualifications

  • Active, unrestricted Physical Therapist (PT) license in the state of assignment
  • Current BLS certification (AHA)
  • 1+ year of recent home health experience (within the last 2 years)
  • Experience conducting PT evaluations and developing therapy plans
  • Familiar with Medicare-certified home health guidelines and OASIS-E documentation
  • Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
  • Clear background check, driving record, and up-to-date health clearance (TB, immunizations)
  • Reliable transportation with valid auto insurance and ability to travel between patient homes
