Travel Home Health Physical Therapist - $2,310 per week

Conduct one-on-one, evidence-based care for adult and geriatric patients in their homes in Albany, Kentucky, including comprehensive evaluations, individualized treatment plans, and interventions to improve functional mobility and safety. Perform in-home assessments, create and progress home exercise programs, provide gait and balance training, and educate patients and caregivers on safe transfers and assistive devices. Coordinate care with nurses, occupational therapists, and primary care providers while managing fall prevention, post-operative rehabilitation, and discharge planning. Work daytime weekday shifts with same-day electronic documentation, supporting interdisciplinary collaboration and community-based rehabilitation.
