Job Summary
Travel Home Health Physical Therapist needed in Hinesville, GA for a 13-week contract. Provide patient-centered home health PT services, conduct initial and ongoing assessments, develop plans of care, implement interventions for strength, endurance, pain, gait/balance assessments, home safety, patient/caregiver education, collaborate with RNs/OTs/SLPs/physicians, and document per Medicare/OASIS-E and agency standards. Requires PT license in the assignment state, BLS, 1+ year home health experience, EMR experience (Homecare Homebase/NetSmart/Epic), ability to travel between patient homes, valid auto insurance, and up-to-date health clearance. Required Qualifications
- Active, unrestricted Physical Therapist (PT) license in the state of assignment
- Current BLS certification (AHA)
- 1+ year of recent home health experience (within the last 2 years)
- Experience conducting PT evaluations and developing therapy plans
- Familiar with Medicare-certified home health guidelines and OASIS-E documentation
- Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
- Clear background check, driving record, and up-to-date health clearance (TB, immunizations)
- Reliable transportation with valid auto insurance and ability to travel between patient homes
