Job Summary
Travel Home Health Physical Therapist to perform initial and ongoing PT assessments, develop individualized plans of care, conduct gait and balance evaluations, home safety assessments, and mobility training in Pueblo, CO and surrounding areas. Collaborates with RNs, OTs, SLPs, and physicians; documents in accordance with Medicare, OASIS-E, and agency standards; proficient with EMRs (Homecare Homebase, NetSmart, Epic). 40 hours per week in a 15-week contract, with travel between patient homes.
Required Qualifications
- Active, unrestricted Physical Therapist (PT) license in the state of assignment
- Current BLS certification (AHA)
- 1+ year of recent home health experience
- Experience conducting PT evaluations and developing therapy plans
- Familiar with Medicare-certified home health guidelines and OASIS-E documentation
- Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
- Clear background check, driving record, and up-to-date health clearance (TB, immunizations)
- Reliable transportation with valid auto insurance and ability to travel between patient homes
