Travel Field Nurse (RN)

Conduct home health nurse visits in Bakersfield, Lancaster, and surrounding So-Cal areas, administering IVs, changing dressings, cleaning wounds, and performing comprehensive patient assessments. Document all findings, physician orders, and treatment plan revisions through the Synergy Software portal while coordinating care with the Clinical Team Manager and other team members. Manage re-certifications, resumption of care, transfers, and discharges as directed, ensuring full compliance with state regulations and Medicare standards. Maintain independence for patients through direct service delivery and regular supervisory visits for Home Health Aides. Availability requires flexibility for weekend and holiday shifts alongside a minimum of two-to-three days per week.
