Job Summary
Coordinate care and services for adult and geriatric patients across the continuum by collaborating with physicians, third-party payers, and the health care team to achieve cost-effective clinical outcomes. Monitor patients using established Utilization Review criteria to ensure efficient resource allocation and manage the interdisciplinary process of assessing, planning, implementing, and measuring intervention effectiveness. Provide care appropriate to patient age while working Sunday through Saturday from 8 a.m. to 5 p.m. with variable shifts. Requires a bachelor's degree in nursing or social work, with certification beneficial.
Required Qualifications
- Graduate of an accredited school of nursing or social work
- Minimum bachelor's degree
- Case Management certification
