Responsible for coordinating transportation of residents to and from physician appointments and inpatient/outpatient programs, developing driving schedules based on appointments, transporting residents between hospital discharge and center, and maintaining safety and vehicle condition. Duties include reporting changes in resident status, transporting medical records, assisting nursing/clinical staff with lifting and transfers, establishing communications with outside facilities, and ensuring timely scheduling and hydration for residents. Requires a valid driver's license and a high school diploma or equivalent; BLS/CPR certification is preferred. Located at Weatherford, TX facilities; weekend-focused, part-time role with emphasis on safety, documentation, and coordination with clinical staff.
