Job Summary
Coordinate transportation for residents to medical appointments, transport residents to and from inpatient and outpatient programs and to and from facilities and residences, develop driving schedules based on appointment needs, assist with safe lifting and transferring as directed by clinicians, maintain a clean and safe van, transport medical records for signatures, and communicate with outside facilities to ensure proper services. Requires Nurse Aide certification with State Registry in good standing and current BLS/CPR certification. Maintain safety policies, document changes in resident condition, and provide hydration per resident needs.
Required Qualifications
- Certified in the state of practice
- Successful completion of State Approved Nurse Aide Training and Competency Evaluation Program; Name must appear on State Registry as Nurse Aide in good standing.
- Current BLS/CPR certification (All SNFs; ALFs as required by state).
